We can assist you in navigating life’s transitions and attaining your objectives with proper planning and networking.
Our expert team provide assistance in coordinating or managing life stages, transitions and supports. Our Life Stage Transition programme provides short- and long-term assistance with a focus on assisting you to live at home, participate in the community, and assist you in coordinating your own support. This includes coordination and support connection, Assistance with accommodation, tenancy obligations, peer support, Individual skill development and Assistance with decision making and planning.
